END OF DAY REPORT POLICY

Date: November 1, 2017 Policy: End of Day Report Purpose:

To establish appropriate guidelines for an End of Day report. The End of Day report should be emailed at the end of the respiratory therapist’s shift to the facility administrator, DON, ADON and CC’d to the regional manager.

Process Design:

  • All RHIP center RTs will be required to create an end of day report listing all the patients they saw for the day and email it to the Admin., DON, and anyone elses at the center requesting it.
  • Each RT is required to create an email at the end of each day at the center
  • The regional manager will also be CC’d on the email
  • The email will detail who was seen, what was accomplished, any interventions that are recommended and change of status information that needs to be initiated

Start Date:

  • 12/1/2017 for all RHIP Centers

Operational Policy:

  • Include any new admits seen that day with the following information
    • Room Number
    • Resident Name
    • Program that resident is being seen in including frequency
    • Recommendations discussed with residents’ nurse
  • Include any follow up visits that required residents’ nurse being notified and recommendations with the following information
    • Room Number
    • Resident Name
    • Program that resident is being seen in including frequency
    • Recommendations/issues that were discussed with residents’ nurse
    • Any recommendations that were not initiated by nursing or medical staff
  • Include Pulmonary Rehab visits with the following information
    • Room Number
    • Resident Name
    • Program that resident is being seen in including frequency
    • Pulmonary rehab activity participated in including time and if there were any issues regarding increased HR, RR, decrease in SpO2, increase or decrease in BP and or dyspnea
  • Include 30-Day Discharge with the following information
    • Room Number
    • Resident Name
    • Reason for discharge
    • If RT feels resident should continue to be seen as RT complex include why and request for DON to sign off on Request for RT Service form
  • Include any issues, concerns and next visit
    • Equipment
    • Expected day and time RT will be back at facility

Operational Controls:

  • Every end of day report will be emailed using pelvip email only
  • End of day report will be sent to Admin. DON and CC regional manager in charge of center
  • Non-compliance by the RT will result in disciplinary action after the 3rd warning
  • Disciplanary action includes oral & written warnings and possible termination if compliance is not met by RTs
  • Any change to the Admin. And DON will need to be communicated from the RT to the regional manager and vice versa
  • Regional Managers will be held responsible for compliance
